JOB PURPOSE:

To provide support to people in a way that is respectful, promotes independence and enables the individuals you support to realise their own potential. This may also include supporting individuals in accessing the community, in their own homes or in any setting their needs and choices may require. A therapy Co-ordinator will both work alone in conjunction with the people they support and as part of the wider interdisciplinary team (IDT).

Therapy Co-ordinators work alongside the Therapy teams coordinating and implementing therapy plans across the service; developing, writing and monitoring goals, will maintain accurate records and actively ensure effective communication and reporting across all team members both verbally and in writing,

KEY RESULT AREAS:

  • Communicating with the individuals you support and key people using appropriate verbal and written communication methods including tact, empathy and diplomacy. Understanding clinically based information (written and verbal) from involved therapists and communicating in clear and understandable language back to people supported and service team members as needed. This may include using specialist communication methods for which training would be provided
  • Plan and implement activity schedules to include purposeful and meaningful structure which enables the individual’s supports to work on their therapy goals throughout day to day activities and during specialised groups and sessions. Monitoring and updating these schedules will be done routinely to make sure goals are addressed in the most effective and meaningful way possible. Other staff will be encouraged to use and input into schedules and programming will be developed to guide staff in both individual and group work in relation to activities and goal attainment. This will include the use of specialist equipment and assistive technology
  • Recording, completing and overseeing any appropriate documentation in order to ensure that individuals’ records, observations or research results are accurate and informative; this will include the development and collation of SASNOS, OAS-MNR and GAS goals. Collaboration with external and internal therapy providers will be needed through both verbal and written means. Through collation and analysis of this information, the Therapy Coordinator will complete the documentation to evidence outcomes and progress in a person’s various skill based areas.
  • Working as part of an interdisciplinary team, analyse the effectiveness of therapy plans through observation and review of data collected and initiate necessary changes, the Therapy Coordinator will present key information to others, in feedback sessions and within formal review meetings. . to ensure a consistent and effective person-centred approach.
  • Attending, participating and contributing to team meetings on a monthly basis in order to ensure that all issues relating to the Therapy provision of the service are fully understood, and to promote communication both within the service and within the IDT.
  • Planning and organizing quarterly review meetings will all relevant parties including the Registered Manager, therapy practitioners and any other relevant people. Preparation of the paperwork will be required and completion of documentation in a timely manner, ready to present to all parties. This is in addition to planning other relevant meetings to ensure therapy needs are being met.
  • Assisting and monitoring individuals with household activities such as cooking, mealtimes, laundering and cleaning in order to promote a healthy environment for the people you support and the promotion of independent living. Will work with the staff team to make sure that the way all staff work with the person supported is done in a consistent and effective manner.
  • Undertaking all training, statutory and specific, relevant to the needs of the service and the people you support and according to relevant current legislation, in order to develop professionally and gain skills beneficial to the individuals. This may include attending conferences as a delegate and representing Voyage Care when a conference exhibitor. Ensure all policies and procedures applicable to the service and the company are followed in order to comply with regulations and good practice
  • To keep the Registered Manager informed of the outcomes or issues that are identified during Therapy Sessions as well as implementation of plans and support guidance in regard to both goal progression and attainment. This includes reporting and referring any staff performance and conduct issues
  • To be responsible to the Registered Manager for the day to day service provision to the service users by producing detailed support guidance that fully reflects the therapy plans produced by the Therapy Team and providing both written and verbal guidance to staff on a day to day basis. The Therapy Coordinator will be responsible for continuous monitoring of goal data and updating plans and goals as needed for the people we support. . In addition, attendance at quarterly therapy review meetings to disseminate information and feeding back to teams will occur.
  • Assisting with individuals’ care, including personal care, toileting and assisting with bathing in order to maintain a high standard of hygiene where the individual requires this. This may include people moving techniques, using hoists and repositioning individuals as required. This may be part of an individual’s therapy programme

  • Carrying out duties in compliance with the Health and Safety at Work Act 1974 and subsequent Health and Safety legislation. Ensure all equipment and technology used is safe and is include on safety check schedules
  • Working a shift pattern which will reflect the visiting therapists’ schedules and working at multiple sites as necessary; this may include weekdays, weekends , wake nights, sleep-in, wake night duty and bank holidays as required.
